Support Coordination

Clarity Care Support Services provides independent, person-centred Support Coordination to help participants understand, implement, and make full use of their NDIS plan. Our role is to build your capacity, strengthen your networks, and ensure your supports align with your goals, preferences, and lifestyle. We work impartially, advocate for participant rights, and assist with navigating services, reviewing progress, and preparing for NDIS plan reviews.

Supports include:

Individual Support Work

We provide high quality, respectful Support Work tailored to each participant’s daily living needs. Our support workers promote independence, safety, and wellbeing by offering personalised assistance in the home and community. Services are delivered with compassion, cultural sensitivity, and a strong commitment to participant choice and control.

Community Participation &Social Inclusion

Clarity Care supports participants to build confidence, social connections, and meaningful community involvement. We design activities based on individual interests, goals, and abilities to promote independence, inclusion, and wellbeing.

Supports include:

  • Attending social groups, classes,
  • Community events
  • Building social skills and confidence

  • Accessing recreational and leisure activities
  • Developing routines and independence in community settings
  • Support to form social connections and explore new opportunities

Participant Onboarding Process

Starting with a new provider should feel clear and comfortable. Our onboarding process is designed to be straightforward and supportive ensuring participants understand their options services and next steps without feeling overwhelmed.

What we do:

We respond to enquiries promptly and take time to listen to the participant’s situation, needs, and priorities. We provide clear information about our services and identify any immediate risks or urgent support needs.

Step 1

Initial Enquiry & Engagement

What we do:

We provide and explain all documentation in plain language, including service agreements, consent and privacy forms, and feedback and complaints information. Participants are encouraged to ask questions and supported to make informed decisions.

Step 2

Consent, Rights & Service Agreement

What we do:

We conduct a detailed intake meeting to understand the participant’s goals, daily living needs, strengths, preferences, cultural considerations, and informal supports.

    Step 3

    Comprehensive Intake & Goal Discussion

    What we do:

    We identify potential risks relating to personal care, community access, mental health, environmental factors, and support delivery. Where required, safety and emergency plans are developed or updated.

    Step 4

    Risk Assessment & Safety Planning

    What we do:

    We translate goals into practical supports, align services with available NDIS funding, and agree on support frequency, duration, and intended outcomes.

    Step 5

    Support Planning & Funding Alignment

    What we do:

    We coordinate supports and connect participants to appropriate services, including:

    • Matching suitable support workers
    • Allied health and specialist providers
    • Community and mainstream supports such as:
      • Companion Cards
      • Travel Access Passes
      • Centrelink assistance
      • Carer Gateway
      • In-home and community care services
    Where support needs extend beyond the scope of a Support Coordinator (e.g. legal matters, systemic advocacy, formal appeals, or rights-based representation), we support participants to connect with independent advocacy services and relevant community advocacy organisations.

    Connecting Supports, Community & Advocacy Services

    What we do:

    Supports commence with clear schedules, communication preferences, and expectations. Participants and families are supported through the transition into services.

    Service Commencement

    What we do:

    We conduct regular checkins, monitor outcomes, respond to feedback, and adjust supports as needs change. We assist with plan reviews and changes of situation and ensure participants are referred to appropriate advocacy services when matters fall outside coordination scope.
